Dear Parents,
If your child is playing any sport this year, they must have a 2020 physical. We will be offering on site physicals Tuesday, July 14th beginning @ 7:30 am for athletes in the gym. The form (below) must be filled out by the parent and signed by the parent prior to the physical. The fee is $10 cash per child. The payment is to be brought with the athlete at the time of the physical. If you are not able to come this day, you may get your physical at your provider's office.
